Please note that you do not need to fill out the “At Office” portion of the COVID-19 Patient Screening form. Your original form will be printed and ready for you to update in office at the time of your appointment.
Prior to your appointment
iOS devices (iPhone, iPad, etc…) may open the form directly on the website. If this happens, please tap the options icon shown in the picture and select Import to Acrobat. You will be able to fill out the form properly from there.
